Required Physical Exams
A physical exam is required by Nebraska law for all students entering early childhood, kindergarten, seventh grade and all students transferring into North Bend Central from out-of-state or country. Required school physical exams for the 2025-2026 school year must be done on or after March 1, 2025.
Students who wish to participate in athletics will also be required to complete a sports physical. Below are the guidelines for each grade level where a physical is needed.
Physical exam for kindergarten or transfer from out-of-state or country:
A physical exam and vision exam is required for all students entering kindergarten or from out-of-state or country. The vision exam may be completed by a health care provider or an optometrist. A dental exam is also recommended. Your child's health care provider will give you the documentation required for a school physical.
Physical exam for Seventh Grade:
A physical exam is required for all students entering into seventh grade without the vision exam requirement. Completing the seventh grade physical exam also meets the requirement for athletic participation in after-school sports. Your child's health care provider will give you the documentation required for a school physical.
Physical exam for Athletic Participation (7th-12th Grades)
Students who wish to participate in after-school sports for 7th-12th grade are required to have a sports physical. The physical can be done on or after May 1, 2025. If your child is participating in competitive athletics, a physical examination by a licensed health provider is required. Please see attached form required by NSAA.
Exemption Waiver
If you do not wish to provide evidence of a physical or vision exam, a signed statement waiving this requirement must be submitted to the school. Contact your school nurse for more information.
Chronic Conditions
North Bend Central serves many students with chronic health conditions. Below are action plans and forms used to meet the health needs of students. A completed plan is required for medication administration in the school setting. It is the responsibility of the parent/guardian to provide updated action plans and/or medication orders annually. These plans must be completed by a health care provider and signed by the parent/guardian. Please work with your healthcare provider and school nurse in developing a specific plan to keep your child safe, healthy, and ready to learn.
Immunization Requirements
The North Bend Central follows the immunization requirements outlined by the Nebraska Department of Health and Human Services. A copy of each student’s immunization record must be presented to the school’s health office prior to attending school. Students who do not meet the state rules and regulations must have a medical waiver or a notarized religious waiver on file. Any student who does not comply with the state immunization requirements shall not be permitted to continue in school until proof of progress towards compliance is received. Below is a list of the required immunizations.
Required Immunizations for Kindergarten-6th grade:
3 doses of DTaP, DTP, DT, or TD vaccine, one given on or after the 4th birthday
3 doses of Polio vaccine
3 doses of pediatric Hepatitis B vaccine
2 doses of MMR or MMRV vaccine, given on or after 12 months of age, separated by at least one month
2 doses of varicella (chickenpox) or MMRV given on or after 12 months of age. If your child has had varicella (chickenpox), they do not need any varicella vaccines. Written documentation must be provided (including month and year) of varicella disease from the parent/guardian or health care provider
Required Immunizations for 7th -12th grade:
3 doses of DTaP, DTP, DT, or TD vaccine, one given on or after the 4th birthday

Health Screenings
Students enrolled in North Bend Central are provided free health screening in accordance with Nebraska State Statute.
Students in PK-4th grade, 7th, and 10th grades are screened for hearing, vision, dental, height and weight per the rules and regulations set forth by the State of Nebraska. The screening is conducted to help identify health concerns in as early a stage as possible, in an effort to enhance learning. Screening is not intended to be diagnostic, but to identify students with potential problems. Screenings are conducted throughout the school year and may begin as early as August.
Per Nebraska State Regulations, a parent/guardian may supersede the mandated school health screening by submitting a statement signed by an authorized health care provider attesting that the required screening has been completed in the previous six months. An updated healthcare provider statement must be submitted each school year.
