If you are unable to come to this event and plan on participating in a sports in the 2024-2025 school year or if you are going into 7th grade, please take THIS form with you to your doctor.
All physicals need to be turned into the school before the start of fall practice.
**Please note that ALL 7th graders need a physical and updated shot records to the school before the first day of school.
